BCBS plans generally cover each level of care below when a clinician documents that you need it and, where required, the plan authorizes it.

Medication-assisted treatment (MAT) and dual diagnosis care are often covered too, though rules vary by plan. At Heartwood, MAT is considered when clinically indicated and integrated with counseling, and co-occurring mental health conditions are treated alongside addiction through our dual diagnosis treatment for men.

You can file an internal appeal within 180 days of the denial, and if that fails, request an independent external review within 4 months (HealthCare.gov). Our admissions team can help you pull together the documentation.
